Thank you for completing your online registration form. Enrollment process for this project will begin the first week of March, 2020 and within the next week you will receive an information package mailed to your home address.

In the meantime, please ensure you have the following available:

• Paper copy of lab results showing your CBC (complete blood count) done within the last 3 months.
• Written report of diagnostic imaging of the joint with osteoarthritis, done within the last 12 months.

IMPORTANT: Each document must show all demographic information (ie. name, birthdate, etc.). Please be sure that all of the pages show a clear header and footer (ie. no missing information on the top or the bottom of the document). Also, please be sure that if the document indicates there is more than one page that all pages are provided. Thank you.

Note: Please do not send the above documents to the clinic. You will receive a formal request from our clinical research coordinator with further instructions on when these documents are to be sent.
